Ideas & Society: ‘Medical Benchmarks And The Myth Of The Universal Patient’

When my daughter was ten and a half months old, she qualified as “wasted,” which UNICEF describes as “the most immediate, visible and life-threatening form of malnutrition.” My wife and I had been trying hard to keep her weight up, and the classification felt like a pronouncement of failure. Her birth weight had been on the lower end of the scale but nothing alarming: six pounds, two ounces. She appeared as a dot on a chart in which colored curves traced optimal growth; fifteenth percentile, we were told. She took well to breast-feeding and, within a month, had jumped to the twentieth percentile, then to the twenty-sixth. We proudly anticipated that her numbers would steadily climb. Then she fell behind again. At four months, she was in the twelfth percentile. At nine and a half, she was below the fifth.

By revealing how our variable bodies respond to a wide range of environments, it challenges us to rethink universal health benchmarks. These standards inform everything from how we define malnutrition and micronutrient deficiencies to how we estimate the risks of growth abnormalities, metabolic disorders, and cardiovascular dysfunction.

Our pediatrician was worried. Ease off the lentils and vegetable smoothies, we were warned; we needed to get more calories into our babe. Ghee, peanut butter—we were to drench her food in these and other fats and wash them down with breast milk and formula. And that’s what we did. When we came back a month later, though, we learned that she had dropped further—and crossed into “wasted” territory.

Maybe in a decade, the one-size-fits-all curves will give way to standards that recognize the different shapes of different populations, and the advice will shift to match. But, for now, we live in the space between two realities—the numbers on a spreadsheet and the child in our arms. ♦

Ideas: ‘Buy, Borrow, Die’

America’s superrich have always found ways to avoid paying taxes, but in recent years, they’ve discovered what might be the mother of all loopholes. It’s a three-step process called “Buy, Borrow, Die,” and it allows people to amass a huge fortune, spend as much of it as they want, and pass the rest—untaxed—on to their heirs. The technique is so cleverly designed that the standard wish list of progressive tax reforms would leave it completely intact.

Step one: buy. The average American derives most of their disposable income from the wages they earn working a job, but the superrich are different. They amass their fortune by buying and owning assets that appreciate. Elon Musk hasn’t taken a traditional salary as CEO of Tesla since 2019; Warren Buffett, the chair of Berkshire Hathaway, has famously kept his salary at $100,000 for more than 40 years. Their wealth consists almost entirely of stock in the companies they’ve built or invested in. The tax-law scholars Edward Fox and Zachary Liscow found that even when you exclude the 400 wealthiest individuals in America, the remaining members of the top 1 percent hold $23 trillion in assets.

In theory, a democratic system, operating on behalf of the majority, should be able to respond by making adjustments that force the rich to pay their fair share. But in a world where money readily translates to political power, voice, and influence, the superrich have virtually endless resources at their disposal to make sure that doesn’t happen. To make society more equal, you need to tax the rich. But to tax the rich, it helps for society to be more equal.

I came across Byung-Chul Han towards the end of the previous decade, while writing a book about the pleasures and discontents of inactivity. My first researches into our culture of overwork and perpetual stimulation soon turned up Han’s The Burnout Society, first published in German in 2010. Han’s descriptions of neoliberalism’s culture of exhaustion hit me with that rare but unmistakable alloy of gratitude and resentment aroused when someone else’s thinking gives precise and fully formed expression to one’s own fumbling intuitions.

Han’s critique of contemporary life centres on its fetish of transparency; the compulsion to self-exposure driven by social media and fleeting celebrity culture; the reduction of selfhood to a series of positive data-points; and the accompanying hostility to the opacity and strangeness of the human being.

At the heart of Han’s conception of a burnout society (Müdigkeitsgesellschaft) is a new paradigm of domination. The industrial society’s worker internalises the imperative to work harder in the form of superego guilt. Sigmund Freud’s superego, a hostile overseer persecuting us from within, comes into being when the infantile psyche internalises the forbidding parent. In other words, the superego has its origin in figures external to us, so that, when it tells us what to do, it is as though we are hearing an order from someone else. The achievement society of our time, Han argues, runs not on superego guilt but ego-ideal positivity – not from a ‘you must’ but a ‘you can’. The ego-ideal is that image of our own perfection once reflected to our infantile selves by our parents’ adoring gaze. It lives in us not as a persecutory other but as a kind of higher version of oneself, a voice of relentless encouragement to do and be more.
